The Inpatient Coder reviews clinical documentation and diagnostic results as appropriate to extract data and apply appropriate ICD-10-CM codes for billing, internal and external reporting, research and regulatory compliance activities. Accurately code inpatient conditions and procedures as documented in the ICD-10-CM Official Guidelines for Coding and Reporting and in the UHDDS to arrive at the most appropriate APR and/or MS-DRG assignment. Assign POA (present on admission) indicators as per official guidelines. Validate discharge disposition code assignment. Resolve error reports associated with the billing process, identify and report error patterns and when necessary assist in the design and implementation of work flow changes to reduce billing errors. This includes abstracting these code assignments according to facility guidelines for both inpatient and outpatient records.

PRINCIPLE RESPONSIBILITIES

· Reviews professional and hospital inpatient medical record documentation and properly identifies and assigns:
· ICD-10-CM and/or PCS codes for all reportable diagnoses and procedures. This includes determining the correct principal diagnosis, co-morbidities and complications, secondary conditions and surgical procedures.
· MS-DRG, APR-DRG
· Present on admission indicators
· HAC (Hospital Acquired conditions) and when required, report through established procedures.
· Discharge Disposition code
· This includes utilizing technical coding principals and APR and/or MS-DRG reimbursement expertise to assign appropriate ICD-10-CM diagnoses and procedures, as well as abstracting these code assignments according to facility guidelines.
· Meet and/or exceed the established IP production standards.
· Meet and/or exceed the established quality standard of 95% accuracy rate while meeting and/or exceeding production standards.
· Reviews daily/weekly system-generated error reports to correct or complete missing data elements.
· Assists in implementing solutions to reduce back-end billing errors. Assist in resolve billing errors as required.
· Queries physicians appropriately when documentation is not black and white and follows up on queries per facility policy
· Utilizes internal (Atos) and qualified external resources to clarify coding policies, guidelines and “gray areas” while adhering to individual client coding policies
· Keeps abreast of coding guidelines by self-study, Atos assigned education, corporate coding meeting attendance, or related in-services. Participates in internal and external quality review meetings, responses, and corrective action development for corporate and outsourced facilities as instructed.

MINIMUM REQUIREMENTS

· Three years or greater of inpatient coding experience in an acute care facility. Teaching facility or Level I or II trauma center experience a plus.
· Credentials to include one or a combination of the following: RHIA, RHIT, and/or CCS. Additional credentials are preferred but not required.
· Ability to code and maintain >95% quality standards and meet productivity standards as documented for each project.
· Advanced knowledge of medical terminology, anatomy and physiology, disease process, pharmacology, complex surgical procedures.
· Advanced knowledge of accepted medical abbreviations and their meanings.
· Advanced knowledge in the use of specialized references such as the ICD-10-CM & PCS code books, CMS Guidelines, medical dictionaries and texts, and medical journals.
· Must have extensive knowledge of Coding Clinic, and all official coding guidelines including UHDDS.
· Advanced knowledge of hospital information systems, encoders and other technology to facilitate a successful virtual work environment while maintaining maximum communication and adhering to HIPAA security standards.
· Advanced knowledge of Excel, Word and Outlook functions.
· Abides by the standards of Ethical Coding as set forth by the American Health information Management Association (AHIMA) and adheres to all official coding guidelines.

PREFERRED SKILLS

· Primarily remote position therefore does not need a central location.
· Access to high-speed internet and be in compliance with Atos Remote Work Agreement specifications.
· Basic personal computing skills including MS Outlook, MS Word, MS Excel
· Technical skills required to learn and navigate a variety of software systems, trouble-shoot computer problems, install periodic updates to software programs and work efficiently in a virtual environment.
· Strong written and verbal communication skills
· Ability to think/work independently, yet interact positively with team
· Advanced problem-solving skills
· Familiarity with current healthcare based technology, coding, and EHR.
· Attention to detail is crucial to this position.
