JOB TITLE: Adult Social Care Broker
LOCATION: Working from home, or at the office in Kingston when safe to travel
PAY RATE: Up to £20.71 PAYE / £26 Umbrella per hour
START DATE: ASAP
DURATION: 3 months minimum
Main Duties/Responsibilities:
· Collaborating with key internal and external partners such as CCG, Hospital discharge, social workers, community teams, voluntary sector and service providers
· Leading on the brokerage of care packages and promotes independent living, reflecting the requirements of care plan
· Commissioning services in line with financial and contractual requirements that provide value for money and which are sustainable
· Maintaining and ensuring records and systems are updated accurately and are GDPR compliant with regard to vacancies, allocations and allocations across Adult Social Care
· Undertaking horizon scanning to ensure best practice
· Collaborating with service users/LPA/professionals and others to support the implementation of care plans effectively
· Leading on the communication of the Kingston Offer to service users/families and monitors the number of offers per person
· Recording purchasing activity within Brokerage systems and IAS and extracts information and data for reporting purposes
· Ensuring that contracts and Service Level Agreements are signed, returned and implemented appropriately
· Monitoring quality assurance of service, escalating any concerns with internal commissioners
Experience required:
· Experience of working an adult social care setting
· Experience of brokering service for vulnerable people and an awareness of the the Care Act
· Experience of working with different key internal and external partners, incl. as Social workers, finance, hospital discharge and community teams, care providers
· Good negotiation skill, conflict management skill, flexible approach, work on own initiative,
MUST BE ABLE TO WORK 7/7 (including weekends on a rota basis). Service Operating Hours 8am -6pm.
